By Participating in a mini-table session you agree that: 


There are inherent risks with any hands-on services including, but not limited to, massage therapy. By participating in today’s session,  you waive claims for damages or injuries, that you may have against the event sponsor, Creative Wellness Strategies and the massage therapist. It is your responsibility to alert the therapist of any injuries, conditions, health status or areas of concern before your session starts. During your session, it is your responsibility to communicate with the therapist if you experience any pain or discomfort.
 

  1. A licensed physician has verified your good health and physical condition to participate in such services.
     

  2. You will alert your Creative Wellness therapist of any medical conditions or physical limitations before your session, including if you are post-surgical, pregnant, or postnatal.
     

  3. You alone are responsible to decide whether to participate and participation is at your own risk.
     

  4. You hereby agree to irrevocably release and waive any claims that you have now or may have hereafter against the even sponsor, Creative Wellness Strategies, and its therapists.
